The Silent Killer: Why Hypertension Is Dangerous
High blood pressure (hypertension) earns its nickname "the silent killer" because it typically causes no symptoms until it has already caused serious, sometimes irreversible, damage. In India, an estimated 200โ250 million adults have hypertension โ but only about half know they have it, and far fewer have it under control.
Left untreated, hypertension is a leading cause of heart attack, stroke, kidney failure, and vision loss. The good news: it is highly manageable with the right interventions.
Understanding Blood Pressure
Blood pressure is the force of blood pushing against arterial walls. It is measured in two numbers:
- Systolic pressure (top number): pressure when the heart beats and pumps blood
- Diastolic pressure (bottom number): pressure when the heart rests between beats
Blood pressure is measured in millimetres of mercury (mmHg).

Causes of Hypertension
Primary (Essential) Hypertension
The most common type (90โ95% of cases). No single cause; instead, a combination of:
- Genetics and family history
- Age (arteries stiffen with age)
- Excess weight and obesity
- Physical inactivity
- High sodium diet
- Excessive alcohol
- Chronic stress
- Tobacco use
Secondary Hypertension
Caused by an underlying medical condition:
- Kidney disease (most common cause of secondary HTN)
- Adrenal gland tumours (phaeochromocytoma, Conn's syndrome)
- Thyroid disorders
- Sleep apnoea
- Certain medications (some oral contraceptives, NSAIDs, decongestants)
The Indian Context: Why It's Especially Relevant
Indians face a unique risk profile:
- Dietary habits: Indian diets are often high in sodium (salt added in cooking, pickles, papads, processed foods)
- Genetic predisposition: South Asians may be more sodium-sensitive
- High prevalence of diabetes: The two conditions frequently co-exist and dramatically increase cardiovascular risk
- Stress and urbanisation: Rapidly changing lifestyles contribute to hypertension risk
- Low awareness: A 2023 survey found fewer than half of Indian adults with hypertension were aware of their condition
Lifestyle Management: The First-Line Treatment
For Stage 1 hypertension and many Stage 2 cases, lifestyle modifications alone can normalise blood pressure. Even if medication is required, lifestyle changes enhance its effectiveness.
The DASH Diet
The Dietary Approaches to Stop Hypertension (DASH) diet is specifically designed to lower blood pressure:
- Rich in: Fruits, vegetables, whole grains, legumes, low-fat dairy, nuts
- Limited: Red meat, sugar, saturated fat
- Very limited: Sodium (aim for less than 2300 mg/day; ideally 1500 mg/day for hypertensives)
Sodium Reduction
Reducing sodium intake by 1000 mg/day can lower systolic BP by 5โ6 mmHg. In practice, this means:
- Removing the salt shaker from the table
- Reducing pickles, papads, and high-sodium condiments
- Reading labels on packaged foods
- Cooking with minimal salt
Physical Activity
Regular aerobic exercise lowers systolic BP by 5โ8 mmHg:
- Aim for 150 minutes/week of moderate activity
- Brisk walking, cycling, swimming, or any activity you enjoy
- Even brief (10-minute) exercise sessions count
Weight Management
Losing 5โ10 kg in overweight individuals can reduce systolic BP by 5โ20 mmHg.
Reducing Alcohol
Heavy drinking raises blood pressure significantly. Limit to one drink/day for women, two for men.
Stopping Smoking
Smoking causes an acute blood pressure spike and long-term vascular damage. Quitting reduces cardiovascular risk substantially.
Stress Management
Chronic stress contributes to hypertension. Evidence-based approaches include:
- Yoga and meditation (several studies show modest BP reduction)
- Adequate sleep (7โ9 hours/night)
- Cognitive behavioural therapy for chronic stress
- Regular leisure activities and social connection
Monitoring Your Blood Pressure at Home
Home blood pressure monitoring is recommended for:
- Confirming hypertension diagnosis (ruling out "white coat" hypertension)
- Monitoring treatment effectiveness
- Improving long-term blood pressure control
Use a validated automatic upper arm monitor. Measure at the same time each day (ideally morning and evening), after 5 minutes of rest, twice per reading. Record your results and share with your doctor.
When Medication Is Needed
If lifestyle changes are insufficient to control blood pressure after 3โ6 months, or if the patient has high cardiovascular risk, comorbidities (diabetes, kidney disease), or very high blood pressure, medication is indicated.
Common drug classes:
- ACE inhibitors / ARBs: Preferred for patients with diabetes or kidney disease
- Calcium channel blockers: Effective in elderly patients and Africans/South Asians
- Thiazide diuretics: Good first-line option
- Beta-blockers: Used when there is also heart failure or coronary disease
Many patients require two or more drugs to achieve target blood pressure. This is not a sign of failure โ it reflects the complex physiology of blood pressure regulation.
The Importance of Regular Monitoring
Hypertension is a chronic condition. Once controlled, regular check-ups are essential:
- Blood pressure check every 3โ6 months
- Annual kidney function tests (eGFR, urine albumin)
- Annual cholesterol and blood glucose
- Regular eye examinations (hypertension damages blood vessels in the retina)
